Policies
When you arrive for your first appointment, you will be
required to fill out a brief intake form and sign the
policies page as seen below.
If
you’d like to save time for when you arrive, you may
download and print both --the intake
form and this policy page and sign to bring in with you
on your first appointment. Otherwise, you may fill both
out at my office..

Back In Balance Massage & Wellness Policies
Cancellations-
As a professional courtesy, I require 24 hours notice
if you need to cancel or reschedule. This may be
waived in the case of illness, inclement weather or
extreme emergency. Less than 24 hours, there is a $30
charge which will be collected at your next scheduled
appointment time. Likewise, if I need to cancel with
less than 24 hours notice, I will add 20 minutes to your
next scheduled, paid appointment. “Repeat offenders”
will be required to prepay for their appointment. Your
consideration and respectfulness is much appreciated!
Referrals-
My business grows through your referrals. To show my
appreciation, each time you refer someone to me, you
will receive an additional free 30 minutes to
your paid appointment of 60 minutes or longer.
Payment-
Cash payment is always appreciated and preferred,
however, checks and Visa/Mastercard are accepted. If you
do pay by check, please pre-write the check before your
appointment. At
Maple Ave, endorse it to Nita Keesler. At Global
Fitness, endorse it to Global Fitness.
There is a $25 returned check fee. For Credit Card
payment, I will need you to bring your card with you.
Sorry, I do not accept health insurance, but I can issue
you a receipt if you would like to check with your
insurance carrier for reimbursement.
Packages- You have the option of
pre-paying for your massage appointments and gift
certificates in a package. For Packages, please
pay in cash or credit only. (**Please note: For
your own massage package, it is recommended to receive
all 6 massages within 3-6 months for optimal health
benefits to create healing benefits for your body.
Gift Certificates—May be redeemed at
Maple Ave. location only.
Arrival- Please arrive 5-10 minutes
before your appointment so that your session may begin
on time, and to allow for
traffic.
Intake/History Form- Every client will
fill out an intake form/history on the first visit. It
is the client’s responsibility to keep the therapist
updated on any changes in health. Information is
strictly confidential.
Water-
It is STRONGLY recommended that you drink at least 2-
8oz. glasses of water within an hour after your massage,
preferably more in the subsequent hours. This should
prevent the possibility of feeling tired or sore or any
other possible after effects of massage.
Cologne/Fragrances-
Please refrain from wearing perfume or cologne
prior to your massage appointment due to
possible allergies of other clients as well as your
therapist.
Jewelry-
Please remove jewelry before your massage, especially
necklaces and dangling earrings. I can massage around
rings.
Termination of
massage-Client understands that this
service is strictly non-sexual and non-sensual in
nature, and that any illicit or sexually suggestive
remarks or gestures will result in the IMMEDIATE
termination of massage session. Client also understands
that massage cannot be provided if he/she is
intoxicated. It is required to pay for the session in
full.
